Three ways people arrive at this clinic
Whichever one you are, the first job is the same. Work out what is actually happening before anyone starts treating it.
You have just been diagnosed
A positive breath test and not much explanation. You want to know what the result means, which subtype you have, and what happens now, before you start taking anything.
Starting well matters more than starting fast. Getting the subtype and the driver right at the beginning is what stops this becoming a three year project.
You have been treated, and it came back
You have done the herbals, or the rifaximin, or both. You cut the foods and followed the protocol. You improved, and then within months you slid back.
This is the most common presentation here. It is a treatable pattern, and it usually means the overgrowth was addressed and nothing else was.
You do not have a diagnosis at all
Years of bloating, unpredictable bowels and a food list that keeps shrinking. Tests come back normal. You have been told it is IBS, or stress, and left there.
You do not need a label before you are allowed to be investigated properly.
You might recognise some of this
- Your bloating is worse at the end of the day than the start, and it has a shape you can see
- You have completed one or more rounds of antimicrobials, improved, then slid back within months
- Your food list has only ever got shorter
- Probiotics or fibre make you worse, which everyone finds confusing
- You have been told your results are borderline and left without a plan
- You have iron or B12 that will not hold, despite supplementing
- Someone has suggested it is stress, and you have stopped arguing
None of this means your case is exceptionally complicated. It usually means the treatment addressed the overgrowth and stopped there.
Why SIBO keeps coming back
SIBO is not a random event. It is what happens when the conditions in the small intestine allow bacteria to accumulate where they should not.
Those conditions are usually one of a short list. Impaired motility, most often after food poisoning. Low stomach acid or bile insufficiency. Structural change from surgery or adhesions. Medications that slow the gut. An immune system that is not clearing organisms the way it should.
Antimicrobials reduce the overgrowth. They do not change any of those conditions.
That is the entire reason relapse rates are high. The bowel is returned to exactly the state that produced the overgrowth in the first place, with a microbiome that is now more depleted than when you started.
Most protocols focus on killing. Very few focus on rebuilding.
The Feed Forward methodology is the approach I developed to close that gap. It treats the overgrowth, then does the part that gets skipped: restoring motility, rebuilding microbial diversity, repairing the mucosal barrier, and widening the diet rather than narrowing it further.
You can clear SIBO and still be unwell. Restoration is what makes the clearance hold.
How SIBO treatment works at Nourished Gut Clinic
Everything happens by telehealth, wherever you are in Australia. The structure is the same for every client.
1. Discover
A full history, current and past testing reviewed properly, and blood pathology so we can see iron, thyroid, inflammatory and nutritional markers alongside your gut picture.
Where breath testing has not been done, or was done without methane or hydrogen sulphide, we run it.
2. Design
A treatment plan built around your subtype, your history and what you have already tried.
Antimicrobial approach, motility support, dietary staging and mucosal repair are sequenced deliberately, not stacked all at once.
3. Evolve
Retesting, reintroduction and progression.
This is the stage most people have never reached, and it is where lasting change actually happens.
Six months is the timeframe because that is what restoration takes. Treatment is a fraction of it. The rest is rebuilding. The full structure sits inside the Nourished Gut Program.
SIBO testing, and why the subtype changes everything
Breath testing measures the gases produced when bacteria in your small intestine ferment a test substrate. Which gas comes up determines the treatment.
Hydrogen
Typically associated with looser stools and urgency. Produced by bacteria fermenting carbohydrate in the small intestine.
Methane, or IMO
Produced by archaea rather than bacteria, which is why it is now called intestinal methanogen overgrowth. Strongly associated with constipation.
It responds to different agents than hydrogen does. Treating it as though it were hydrogen SIBO is one of the most common reasons a protocol fails.
Hydrogen sulphide
A third gas, associated with diarrhoea, sulphur intolerance and a distinct symptom picture. It is not currently measurable by breath testing in Australia, and the diagnostic criteria for it have not yet been validated.
That does not mean it is ignored. Where the pattern fits, it is recognised and treated clinically, based on your presentation rather than a number on a report.
Fructose as a testing substrate
In my own practice I also test using fructose. It identifies fructose malabsorption, and it distinguishes malabsorption that is primary from malabsorption that is secondary to an overgrowth. Those two situations need different plans, and running only lactulose or glucose will not tell you which one you are dealing with.
A test is only worth running if the result changes what you do next. That is the standard I apply before recommending any of it.

The SIBO Restore Diet: restoration, not restriction
Most people arrive here already restricted. Low FODMAP for eighteen months. Specific carbohydrate diet. Elemental. A list of safe foods that fits on one hand.
Monash University, who developed the low FODMAP diet, designed the elimination phase to run for two to six weeks before structured reintroduction. Almost nobody I see has been moved through that reintroduction. They were handed the restriction and left there.
Prolonged restriction reduces the fermentable fibres that beneficial bacteria depend on. The microbiome contracts. Tolerance narrows. The diet that helped in month one is contributing to the problem by month twelve.
The SIBO Restore Diet is a five stage restoration framework, ending in a maintenance pattern called Digestive Resilience. Stage one calms symptoms. Every stage after that adds food back.
It is used only when it is clinically indicated, and it is staged to your presentation rather than applied as a standard protocol. The measure of success is a wider diet at the end than at the beginning.
You shouldn't have to fear healthy food.
